A plan for the SMLE should start from the one thing SCFHS publishes that most plans ignore: the blueprint. Medicine is 30% of the exam, obstetrics–gynaecology 25%, paediatrics 25% and surgery 20% — so half the questions come from two subjects that many Indian graduates have not touched since internship. This 12-week plan follows those weights, uses SCFHS's mastery levels to decide depth, and ends in the October, November or December 2026 windows.

Our split of a 20-hour week over 12 weeks: the four disciplines at slightly under their blueprint weights, leaving time for ethics and biostatistics and for timed mocks.

The four disciplines take 92% of the time in close proportion to the blueprint. The rest goes to the patient safety, preventive medicine and ethics outline (its questions sit inside the four weights, but its topics — screening statistics, research ethics, medication and transfusion safety — need their own hours) and to full-length timed practice, which is the only way to rehearse two 120-minute blocks.

Our plan. With fewer weeks, keep the order and compress — but keep both full mocks, because nothing else practises the 30-minute break and the fourth hour.

Getting the most from the two mocks

The two full-length mocks in weeks 6 and 11 are where most of the plan's value is found. Sit each one as the real thing: two 100-question blocks of 120 minutes, a single 30-minute break, no phone. Then spend at least as long reviewing as you spent sitting, sorting every wrong answer into one of four piles.

Our method. If most of your errors fall in the first pile, you need more content; if they fall in the last three, more questions will help more than more reading.

If you have been out of clinical work for years

Doctors returning to exams after several years in administration, research or a non-clinical job usually find two things: management guidelines have moved on, and the pace of 72 seconds a question feels fast. Start with the timed baseline block in week 1 anyway — it is the fastest way to find which disciplines have faded — and consider stretching the plan to 16 weeks by giving medicine and obstetrics an extra week each. Use the reference-list editions SCFHS names rather than older copies on your shelf, since those editions are what its questions were written against.

Our pacing, from 120 minutes per 100-question block (72 seconds a question). Up to a tenth of the questions are unscored pilot items that you cannot identify, so give each one the same effort. Over-running the 30-minute break is taken off your second block.

In the final week, stop adding sources. Re-read your mock error piles, run one or two timed 100-question blocks to keep the pace, and settle the logistics: the name on your passport must match your Prometric scheduling permit exactly, centres open at 7:30 a.m., and arriving more than 30 minutes late counts as an attempt.

Mistakes that cost marks

  • Studying from a pre-2023 plan. The format is 200 questions in two blocks; plans built for 300 questions over six hours over-train endurance and under-train pace.
  • Treating OB/GYN and paediatrics as minor subjects. Together they are half the exam.
  • Aiming for a percentage. The pass is a scaled 560; the SMLE publishes no percentage equivalent, so aim clearly above the line in your mocks instead.
  • Skipping biostatistics. Screening test parameters and ethics are short to learn and appear in the outline.
  • Only reading. The exam tests interpretation and decision-making through scenarios; practise with questions from week one.

SCFHS also offers an official SMLE practice test drawn from the SMLE item bank, which you apply for on its website; Prometric runs it at a centre or by remote proctoring and emails the score within 24 hours. Its fee is not published. It is the closest thing to the real item style — a good use for week 11.

Practise it, don't just read it

Weeks 4–8 are paediatrics and OB/GYN. Topic quizzes after each week show whether it has stuck.

How many months are needed to prepare for the SMLE?

There is no official figure. A doctor recently out of internship can cover the blueprint in about 12 weeks at 20 hours a week (about 240 hours); someone years from clinical work may need longer.

Which subject has the most weight in the SMLE?

Medicine, at 30%. But obstetrics–gynaecology and paediatrics are 25% each, so together they are half the exam; surgery is 20%.

What are the high-yield topics for the SMLE?

Start with the level-3 topics in the SCFHS outline: coronary disease, heart failure, asthma, COPD, diabetes and its emergencies, thyroid, electrolytes, sickle cell disease, pneumonia, TB, immunisation, development, croup, nephrotic syndrome and patient-safety topics.

Is there an official SMLE practice exam?

Yes. SCFHS offers an SMLE practice test sampled from its item bank. Apply on the SCFHS website; Prometric runs it at a centre or remotely and sends the score within 24 hours.

How much time do I get per question in the SMLE?

120 minutes for each block of 100 questions — 72 seconds a question.
