SMLE for Indian MBBS: Eligibility, Internship and the One-Year Rule

When an Indian MBBS student, intern or graduate can sit the SMLE, why sitting is not the same as being classified, how the 12-month experience rule works, and which title an MBBS leads to.

SMLE for Indian MBBS: Eligibility, Internship and the One-Year Rule

Checked 25 September 2026. From the SMLE Blueprint 2026, SCFHS classification requirements and the Membership Regulation criteria (Aug 2025). SCFHS decides each file; no SCFHS document names the Indian internship.

Two different rules decide an Indian doctor's route to Saudi Arabia, and most confusion comes from mixing them up. One decides who may sit the SMLE — and it is generous: interns can, and so can students a year from graduating. The other decides who SCFHS will classify as a practitioner — and for a doctor trained outside Saudi Arabia, it asks for at least a year of experience. This guide separates the two, from SCFHS's own documents.

Where other pages get this wrong

You do not need post-internship experience to sit the SMLE — only to be classified afterwards.

Commonly published instead: "At least one year of post-internship clinical experience is required to take the SMLE."

SCFHS's 2026 blueprint lets you sit with a recognised MBBS or equivalent, once you have started your internship year, or as a student one year away from graduation. The 12-month experience rule is in SCFHS's classification requirements and its August 2025 membership criteria: it applies when SCFHS classifies a non-Saudi practitioner, not when you book the exam.

The practical meaning for an Indian doctor: you can take the SMLE during your internship and have the result in hand, but SCFHS will not classify you as a General Practitioner until you can show a year of experience. Passing early is not wasted — the result is what SCFHS and the Saudi Board matching use — but plan the job search around the classification rule, not the exam.

Does the Indian internship count as the year?

No SCFHS document mentions the Indian compulsory rotating internship. Two official rules point the same way: the only internship-based exemption from the 12 months is an internship done entirely inside Saudi Arabia, and the Saudi Board matching rules say experience "must have been earned after the internship period". SCFHS also asks for the internship certificate as a separate document from experience letters. The safe reading is therefore 12 months of experience after your internship. It is a reading, not a ruling; if your case is borderline, ask SCFHS through Mumaris+ before relying on it.

Our illustration, combining SCFHS's rules with the NMC's 12-month internship (CRMI Regulations 2021, reported from a government copy of the gazette). A doctor who sits the SMLE in the internship year can therefore have a pass in hand a year or more before classification — useful, since a pass also feeds residency matching.

What counts as experience

SCFHS's membership criteria (August 2025) say experience must be in the practitioner's own specialty, and the professional councils set its nature and minimum length. For an MBBS doctor seeking the General Practitioner title, that means clinical work as a doctor after the internship; an experience letter must show at least a year, backed by an employment letter issued within the last three months. A break in practice does not automatically disqualify you: the criteria deal with it through extra training, an assessment or CPD, decided case by case. SCFHS also reserves the right to exempt categories of practitioners from the experience rule in the public interest.

Experience is verified like qualifications — by SCFHS contacting the issuing body, checking its official website, or through an SCFHS-approved verifier such as DataFlow. If the issuing body does not respond within 20 working days, SCFHS records it as unable to verify and the application is rejected (procedural guide, August 2025). Work that the hospital that employed you cannot confirm will not help, however real it was.

Where other pages get this wrong

Housemanship abroad is not the rule for Indian graduates — the Indian internship is the internship.

Commonly published instead: "Complete at least one year of post-graduation internship or housemanship before applying."

For an Indian MBBS, the internship SCFHS asks for is the NMC compulsory rotating internship, submitted as its own certificate. The separate year SCFHS requires of non-Saudis is experience, not a second internship, and it is a condition for classification — not for sitting the SMLE.

From Appendix B of SCFHS's membership criteria (August 2025, in Arabic), which we have translated. Whether an Indian MD or MS is treated as master's-level is SCFHS's decision for each qualification; the specialist route may use an evaluation rather than the SMLE. The criteria also exempt a practitioner from professional assessment if SCFHS assessed them in the same specialty within the last five years.

Where are you on the SMLE route?

If you are aiming for residency, not a GP job

The Saudi Board matching rules treat experience differently again. The portfolio (15% of the matching score) has a clinical-experience item that needs at least six months after the internship — continuous, or two blocks of three — in your first-preference specialty, under a supervisor registered with SCFHS (for experience abroad, the supervisor's classification number in that country must be given, and SCFHS may reject experience it cannot confirm). Experience during or before the internship is not considered. The SMLE itself is 55% of the score and remains valid for matching for five years, so an Indian graduate who sits it during internship and passes well keeps that result through several matching cycles. After matching, registering for training requires a valid professional classification, a sponsor letter and the annual training fee — so the classification rules above still apply before you start.

The SMLE can be sat as early as final year; the earlier you start practising, the more of it you can take into that first sitting.

Can an MBBS intern take the SMLE?

Yes. SCFHS's 2026 blueprint lets those who have started their internship year, and students one year away from graduation, sit the SMLE. Its separate final-year note names Saudi university students, so an Indian final-year student should confirm eligibility in Mumaris+ first.

Is one year of experience needed for the SMLE?

Not to sit it. SCFHS requires at least 12 months' experience from non-Saudi doctors trained outside Saudi Arabia when it classifies them as practitioners.

Does the Indian internship count as experience for SCFHS?

No SCFHS document says so. The only internship-based exemption is an internship done entirely inside Saudi Arabia, and the matching rules count experience only after the internship, so plan for 12 months after your internship.

What title does an MBBS doctor get in Saudi Arabia?

Yes. SCFHS says classification can be applied for from outside Saudi Arabia through Mumaris+, without a Saudi work permit; registration needs lawful residence in the Kingdom.
