Checked 23 October 2026. Fees, hours, the appointment list and booklet formats are as the FDNY pages and study booklets stated them on 23 Oct 2026. The FDNY asks candidates to check for the latest booklet before the exam, and notes that some Notices of Exam are being updated for online applications.
An FDNY Certificate of Fitness (COF) is the card the Fire Department of the City of New York issues to people who do certain dangerous jobs: fire guards, torch operators, the people who inspect sprinkler systems and many more. In the FDNY's words, under the New York City Fire Code a COF is needed to "manufacture, store, handle, use, maintain, inspect, test or transport certain materials", to "conduct certain operations" and to "operate certain facilities". For the F-01, F-03/F-04, F-07, F-60 and G-60 certificates the booklets list no alternative issuance procedure: you get one by passing a computer exam at FDNY Headquarters in Brooklyn (with one shortcut: the F-03/F-04 booklet says someone who passes the F-03 may pay an additional $25 to get the F-04 without taking the F-04 exam). This guide sets out the current process from the FDNY's own pages, the format of five common exams, and what renewal costs. ProSyllabus is an independent study site, not part of the FDNY (Fire Department of the City of New York) or the City of New York. Every rule here is quoted or summarised from the FDNY pages and study booklets listed at the foot of the page.
FDNY General Notice of Exam (revised 07/2023), FDNY Certificates of Fitness page and NYC Business F-60 page, read 23 Oct 2026.
How to get a Certificate of Fitness, step by step
The FDNY Certificates of Fitness page says "All Certificate of Fitness (COF) transactions must be done online" and that applicants taking an exam "must complete Certificate of Fitness (COF) application and payment online before coming to FDNY Headquarters." Older FDNY documents still describe in-person steps: the S-12 booklet an in-person application, the 2023 General Notice in-person renewal, and the NYC Business pages submitting materials in person or by mail. The S-12 booklet and several NYC Business pages also list cash as a way to pay, while the General Notice says cash is not accepted. The FDNY Certificates of Fitness page says all transactions are online and cash is no longer accepted; follow that page, and check it before you go.
| Step | Detail |
|---|---|
| 1. Pick the right certificate | Each certificate has its own code (for example F-60, G-60, S-12) and a free Notice of Exam and study booklet, listed alphabetically on the FDNY Certificates of Fitness page. Some certificates and upgrade exams share one booklet (F-03/F-04, F-07/W-07, S-12/S-15 and the T-15 upgrade exam). |
| 2. Create an NYC.ID and apply online | Through the FDNY Business portal: if you do not have an NYC.ID, "create account" first, then log in to make the COF application. |
| 3. Pay the fee online | $25 per attempt (General Notice of Exam). The FDNY page says exam fees can be paid "by credit/debit card, check or money order only" and "Cash is no longer accepted"; the NYC Business page lists credit card, debit card or E-Check for paying online. The General Notice puts a 2% convenience fee on credit card payments, and the NYC Business page says the fee is non-refundable. |
| 4. Gather what to bring | A government-issued photo ID, such as a state driver's license, non-driver ID or passport, to enter Headquarters. Supporting documents go with the online application (the NYC Business page says to gather electronic copies). For in-person applicants the older General Notice lists a letter of recommendation from your employer on letterhead, valid 6 months, or a notarized letter if you are self-employed. |
| 5. Go to FDNY Headquarters | 9 MetroTech Center, Brooklyn. Most COF exams are walk-in, Monday to Friday (except legal holidays), 8:00 AM to 2:30 PM. The FDNY page lists F80, F89, Q01, S56 and S98 as needing an appointment, booked with your COF application ID (the 2023 General Notice lists N-85, N-89/T-89/Z-89, Z-51, S-56 and S-98). Groups of ten or more should contact the COF unit for a group appointment. |
| 6. Take the exam | On a touch-screen computer, time-limited, 70% to pass (the exams in this guide are multiple choice). If you pass and meet all the requirements, the certificate is issued the same day. |
Not working yet? The General Notice says applicants who are not currently employed may take the exam without the employer documentation; if they pass, the FDNY issues a temporary letter with a picture for job-seeking, and the card itself is issued once they are employed and provide the documents. That does not apply where a certificate has a school requirement: then the exam cannot be taken until the school is completed. Applicants must be at least 18 and have a reasonable understanding of English. Fee waivers: the FDNY page says fee exemption applies to NYC city agency employees with a recommendation letter on agency letterhead; the older General Notice speaks of government employees who will use the certificate for work and also asks for a copy of the agency identification card. Testing accommodations under the Americans with Disabilities Act are requested on the FDNY's Test Accommodation Request Form.
What happens in the exam room
| Rule | What the FDNY says |
|---|---|
| Format | Every COF exam is administered on a "touch screen" computer monitor. The exams in this guide are multiple choice; the F-60, F-03/F-04 and F-07/W-07 booklets say each question has four alternatives and only one is most correct |
| Time | "All exams are time-limit exams. The time limited is based on the amount of the questions" |
| Pass mark | "A passing score of at least 70% is required". The F-60, G-60, F-03/F-04, F-07/W-07 and S-12/S-15 booklets add that an unanswered question, or one with more than one answer marked, is scored as incorrect, and that there is no penalty for guessing |
| Study booklet | Study material and booklets may not be used during the exam; only reference material provided by the FDNY may. If an exam needs reference material, exam room staff provide it and the computer prompts you |
| Dictionary | You may bring your own clean paperback dictionary; electronic dictionaries are not allowed |
| Phones | All electronic devices, including cell phones, must be turned off |
| Cheating | You may be barred from taking any COF exam for up to six months and may face criminal charges or civil lawsuits |
| Result | Pass: if all requirements are met, the certificate is issued the same day (someone without the employer documents gets the temporary job-seeking letter instead). Fail: a failure report. Online applicants can see the result (pass or fail) and expiry date online within one or two business days |
| Retest | A new application and payment for each attempt. The F-01, F-03/F-04, F-07/W-07, F-60 and G-60 booklets add: "Retesting on the same calendar day is not permitted" |
The booklets also tell you, in their own words, not to ask staff about the questions: FDNY staff cannot assist applicants with their tests. If the computer has a problem, ask the examiner in the room.
Exam format of the common certificates
Each certificate's study booklet gives its own number of questions and time. The time follows the number of questions, at about one and a half minutes a question for most exams (our arithmetic). Five of these have independent practice exams on ProSyllabus (linked below).
| Certificate | Questions | Time | Correct answers needed (our arithmetic, 70%) | Booklet |
|---|---|---|---|---|
| F-01 Fire Guard for Impairment (Citywide) | 20 | 30 minutes | 14 of 20 | revised January 2026 (contact/payment info) |
| F-03 Indoor Place of Assembly Safety Personnel | 25 | 38 minutes | 18 of 25 | revised January 2026 (contact/payment info) (shared with F-04) |
| F-04 Temporary Place of Assembly Safety Personnel | 20 | 30 minutes | 14 of 20 | revised January 2026 (contact/payment info) (shared with F-03) |
| F-07 / W-07 Fire and Emergency Drill Conductor | 40 | 61 minutes | 28 of 40 | revised January 2026 (contact/payment info) |
| F-60 Fire Guard for Torch Operations | 25 | 38 minutes | 18 of 25 | revised January 2026 (contact/payment info) |
| G-60 Torch Use of Flammable Gas | 35 | 53 minutes | 25 of 35 | revised January 2026 (contact/payment info) |
| S-12 Citywide Sprinkler Systems | 75 | about 135 minutes | 53 of 75 | revised March 2024 (apply/pay) (shared with S-15, T-15) |
| S-15 Foam-Water Sprinkler Systems (Citywide) | 100 | about 175 minutes | 70 of 100 | revised March 2024 (apply/pay) |
| Exam: T-15 (upgrades a valid S-12 to S-15) | 25 | about 38 minutes | 18 of 25 | revised March 2024 (apply/pay) |
Our arithmetic: the time each FDNY study booklet gives, divided by its number of questions (booklets read 23 Oct 2026). These are averages, not FDNY pacing advice.
Two details from the booklets: the F-03 exam covers the whole F-03/F-04 booklet, while the F-04 exam covers it except the section on theaters; and the S-12 exam covers chapters 1 to 16 of the S-12/S-15 booklet. On the S-12 the FDNY provides reference material in the room, including a sprinkler temperature rating table and inspection, testing and maintenance reference guides.
Which certificate do you need?
The codes are easy to mix up, and holding the wrong one does not let you do the job. Here is what the FDNY booklets say about the common fire guard and torch certificates.
| Certificate | Who must hold it | What it does not cover |
|---|---|---|
| F-60 Fire Guard for Torch Operations | The fire guard doing the fire watch for torch operations at construction sites, on any rooftop, or in connection with any torch-applied roofing system operation, and in any building or structure when the torch operation is conducted by a person holding a citywide permit for torch operations | The F-60 booklet says it does not qualify you to serve as a watchperson at a construction site (that is the S-60) or as a fire guard for impairment at construction sites (that is the F-01) |
| G-60 Torch Use of Flammable Gas | The person using the torch with oxygen and a flammable gas (including LPG or CNG) for any hot work. For LPG/CNG torch work on torch-applied roof systems without oxygen, the booklet accepts a G-41, G-42 or G-60 | The torch operator and the fire watch are separate roles; where the fire watch must be an F-60 holder is in the row above. G-60 holders are not required to get the G-41 but may pay an extra fee for it |
| F-01 Citywide Fire Guard for Impairment | The fire watch in any occupancy where a required fire protection system is out of service. For the first four hours of an out-of-service condition affecting no more than 50,000 square feet, the impairment coordinator, or a trained and knowledgeable person capable of fire watch duties and designated by the building owner, may do it instead | A different job from the torch fire watch of the F-60 |
| S-12 Citywide Sprinkler Systems | A person who inspects, tests and maintains sprinkler systems in New York City buildings | Foam-water systems need the S-15 (or the T-15 upgrade for an S-12 holder) |
| F-03 / F-04 Place of Assembly Safety Personnel | F-03: fire safety at an approved place of assembly or temporary place of assembly at a specific location. F-04: temporary place of assembly events at different locations | Separate certificates with separate exams, though they share one booklet. The booklet says someone who passes the F-03 may pay an additional $25 for the F-04 without its exam; confirm with the FDNY when you apply |
Two example questions
The FDNY booklets include only three sample questions, about the exam room itself; the F-01, F-60, G-60 and S-12/S-15 booklets say these show the format of the exam questions, not the content of the real exam. These two are our own, written from the FDNY study booklets in the same four-option style.
Example 1: F-60 Fire Guard for Torch Operations
A contractor who holds a citywide permit for torch operations will use a torch inside an office building. Who must conduct the fire watch?
- (A) Any building employee chosen by the owner
- (B) At least one F-60 fire guard
- (C) A second torch operator from the same crew
- (D) The torch operator, between cuts
Show the answer
Answer: (B) At least one F-60 fire guard. The study material (Certificate of Fitness section and Part 1.3.2) says the fire watch for torch operations in any building, when the work is done by a citywide permit holder, must be conducted by at least one F-60 fire guard.
An original ProSyllabus question written from the FDNY study material, not an FDNY exam question.
Example 2: S-12 Citywide Sprinkler Systems
A tenant asks to install a security camera inside a housing made to look like a sprinkler head, in a space where it could be mistaken for real sprinkler equipment. According to the study material, this is:
- (A) Unlawful, because it looks like fire protection equipment but performs no fire protection function
- (B) Permitted, but only in areas of the building that have no sprinkler protection of their own
- (C) Permitted, as long as the real sprinkler system in the space is complete and fully in service
- (D) Permitted, if the camera installer labels the housing as a camera and registers it with the building owner
Show the answer
Answer: (A) Unlawful, because it looks like fire protection equipment but performs no fire protection function. The study material (Introduction) says it is unlawful to install or maintain a device that looks like fire protection equipment but does not perform a fire protection function where it may be confused with real equipment, and gives a CCTV camera in a sprinkler-head shell as the example.
An original ProSyllabus question written from the FDNY study material, not an FDNY exam question.
How to prepare
Study the scope your booklet gives: the F-01, F-03, F-60 and G-60 booklets say the exam covers the entire booklet; the F-04 exam leaves out the theater section; the S-12 covers chapters 1 to 16 of its booklet. You cannot use the booklet during the exam, so the work is learning its rules well enough to recall them: the distances, the times, who may do what and in which order. Read your certificate's booklet once straight through, then again by part, and test yourself against the clock. The F-60 booklet also tells fire guards to be familiar with NFPA 51B and Fire Code Chapter 35, beyond what the booklet covers.

Renewal
| Item | What the FDNY says |
|---|---|
| How often | Most certificates every three years (some differ; check your booklet) |
| Fee | $15 for most 3-year certificates |
| Reminder | A renewal notice 90 days before expiry (the General Notice says by email; the NYC Business page describes it as a letter), but renewing on time is your responsibility |
| Late | A $25 penalty on top of the renewal fee for renewals made about 90 days to one year after expiry (the General Notice gives both "90 days" and "91 days") |
| Over a year late | Cannot be renewed; you must take a new exam |
| How | The FDNY page says certificates "must be renewed online and by mail": online with the simplified renewal that needs no NYC.ID, or with an NYC.ID. The older General Notice also lists renewal in person. The FDNY may require a re-examination at renewal |
Frequently asked questions
How much does an FDNY Certificate of Fitness cost?
The application fee is $25 for each exam attempt, paid when you apply online (the FDNY page lists credit or debit card, check or money order; the NYC Business page lists card or E-Check); the General Notice puts a 2% convenience fee on credit card payments, and the FDNY page says cash is no longer accepted. Renewal is $15 every three years for most certificates.
Where do I take the COF exam?
At FDNY Headquarters, 9 MetroTech Center, Brooklyn. The FDNY page says most exams are walk-in, Monday to Friday except legal holidays, 8:00 AM to 2:30 PM, after you have applied and paid online, and lists F80, F89, Q01, S56 and S98 as needing an appointment.
What score do I need to pass?
At least 70% on every Certificate of Fitness exam. On a 25-question exam such as the F-60 or F-03 that means 18 correct answers.
Can I use the study booklet during the exam?
No. The booklets say study material may not be used during the exam; only reference material provided by the FDNY may. If an exam needs reference material, the FDNY provides it in the room. You may bring a clean paperback dictionary.
What if I fail?
You get a failure report. To retake, you submit a new application and pay again; the F-01, F-03/F-04, F-07/W-07, F-60 and G-60 booklets say retesting on the same calendar day is not permitted.
What is the difference between the F-60 and the F-01?
The F-60 is the fire guard for torch operations. The F-01 is the fire guard for impairment, when a required fire protection system is out of service. The F-60 booklet says an F-60 is not qualified to serve as a fire guard for impairment at construction sites.
How long is the S-12 exam?
75 multiple-choice questions in about 135 minutes, covering chapters 1 to 16 of the S-12/S-15 study booklet. The FDNY provides reference material in the room.
Do I need a job to take the exam?
Usually not. Applicants who are not currently employed may take the exam without the employer documents; if they pass, the FDNY issues a temporary letter for job-seeking, and the card once they are employed and provide the documents. A certificate with a school requirement cannot be tested until the school is completed.
