Checked 20 October 2026. Format and marking from Ministerial Decree 1005 of 6 August 2026 (the 2026 test). This guide uses the 2026 decree; check the format against the 2027 decree when it is available. A university’s figure from a past year is a snapshot of one published list, not the 2027 cut-off.
If you are planning a second IMAT attempt in 2027, the most useful thing you own is your 2026 answer sheet, which Universitaly shows you together with your score. From it you can count how many questions you got right, wrong and left blank in each section, and under the IMAT marking that is enough to see exactly where your points went. This guide shows how to read it, how many more right answers a target takes, how to split your study time to September 2027, and when to guess. It also has a free, timed 20-question mini-mock in the real format.
Free · 20 questions · 33 minutes 20 seconds
A mini-mock in the real format: five options per question, +1.5 / −0.4 / 0 marking, the five sections in the order the decree lists them, at the 2026 pace (100 minutes for 60 questions). Every answer is explained, and you see your points by section at the end.
DM 1005/2026, articles 2 and 3 (the 2026 test).
The format you are preparing for
This plan is built on the 2026 rules (DM 1005/2026); check it against the 2027 decree when that is available. In 2026 the paper had 60 questions with five options and one correct answer, in 100 minutes, split across five sections. Each section is worth its number of questions times 1.5 points.
| Section | Questions | Maximum points (questions × 1.5) |
|---|---|---|
| Reading skills and knowledge from studies | 4 | 6.0 |
| Logical reasoning and problems | 5 | 7.5 |
| Biology | 23 | 34.5 |
| Chemistry | 15 | 22.5 |
| Physics and mathematics | 13 | 19.5 |
| Total | 60 | 90.0 |
Biology and chemistry are 38 of the 60 questions and 57 of the 90 points. The syllabus for every section is in Allegato A of the decree; it is the list to plan from, not a prep company’s topic list.
Read your 2026 answer sheet section by section
Your total hides two different losses. A wrong answer costs 1.9 points compared with a right one: the 1.5 you did not get plus the 0.4 taken off. A blank costs 1.5. So the same total can come from very different papers, and they need different fixes: wrong answers often come from guessing or misreading, blanks from running out of time or gaps in the syllabus, and going back over each wrong or blank question helps you tell which.
Here is a worked example: a student with 30 right, 16 wrong and 14 blank (our example, not a real candidate).
| Section | Right / wrong / blank | Points | Lost to wrong answers (× 1.9) | Lost to blanks (× 1.5) |
|---|---|---|---|---|
| Reading skills and knowledge from studies | 2 / 1 / 1 | 2.6 | 1.9 | 1.5 |
| Logical reasoning and problems | 2 / 2 / 1 | 2.2 | 3.8 | 1.5 |
| Biology | 13 / 6 / 4 | 17.1 | 11.4 | 6.0 |
| Chemistry | 7 / 4 / 4 | 8.9 | 7.6 | 6.0 |
| Physics and mathematics | 6 / 3 / 4 | 7.8 | 5.7 | 6.0 |
| Total | 30 / 16 / 14 | 38.6 | 30.4 | 21.0 |
This student scored 38.6. Biology lost the most points, 17.4, more than chemistry (13.6) or physics and mathematics (11.7). The plan for this student starts with biology, the section that both lost the most and carries the most points.
Our example: 30 right, 16 wrong, 14 blank. Lost = section maximum minus points scored.
How many more right answers reach a target?
Pick a target score. The gap is the target minus your total. Each wrong answer you turn into a right one gains 1.9 points; each blank you turn into a right one gains 1.5. So the fewest extra right answers you need come from fixing wrong answers, and the most come from filling blanks.
For a target, use the score of the last candidate given a place at your university in a past year, and treat it only as a reference: it is not this year’s cut-off. Each figure comes from one published list in one year, and non-EU candidates residing abroad are ranked separately at each university. Three 2025/26 examples, re-read on the published lists on 20 October 2026:
| University (2025/26, non-EU residing abroad) | Places | Score at the last place offered on the published list |
|---|---|---|
| Sapienza, Rome — Medicine and Surgery "F" | 13 | 65.80 (rank 13) |
| Bologna — Medicine and Surgery | 20 | 70.3 (rank 20) |
| Pavia — Medicine and Surgery | Ranks 1–35 could enrol | 73 (rank 35) |
Against Sapienza’s 65.80, the student in the worked example is 27.2 points short. That takes at least 15 more right answers if they replace wrong answers (15 × 1.9 = 28.5), and at most 18 if they replace the 14 blanks first (14 × 1.5 = 21.0) and then 4 wrong answers (7.6), 28.6 in all. The free gap report at the top of this page does this arithmetic on your own numbers.
Split your study time by section
Two numbers decide how much time a section deserves: how many points it carries, and how many it lost you. A simple, honest split gives each section half its weight in the paper and half its share of your lost points. For the worked example that puts biology first, chemistry second and physics and maths third; logic and reading together get the least time, because they are only 9 questions.
A month-by-month outline to September 2027
- November–March: one pass through every block of Allegato A, weakest sections first. Start with the free mini-mock to set a baseline.
- January onwards: a full timed mock every four to eight weeks, under test conditions: 100 minutes, no calculator (university calls, for example Bologna’s for 2026/27, do not allow calculators in the room), blanks left blank.
- April–June: a second pass through your two weakest sections only, with timed drills.
- July–August: mocks and official past papers a few weeks apart; redo every question you got wrong before checking the answer.
- September: your error log, short timed sets, no new topics in the last week.
The 2026 test was on 29 September 2026 (DM 1005/2026, art. 2(3)). Move the last month to the 2027 date once it is officially confirmed.
How to sit a full mock so it tells you something
A full mock tells you most when it is sat under test conditions: one sitting of 100 minutes, no calculator, no notes, and no looking anything up halfway. Leave a question blank when you cannot rule out any option, exactly as you would on the day, because the blanks and wrong answers are what you will count afterwards.
Then mark it the way CINECA does: +1.5, −0.4 and 0, section by section. Write down, for every question you missed, which of three things went wrong: you did not know the fact, you knew it but misread or slipped, or you ran out of time. The first is a syllabus gap for your plan, the second is a checking habit, the third is pacing. Keep these in one error log for the whole year, and before the next mock redo every logged question without looking at the answer. Comparing section points from one mock to the next tells you more than the total does, because the total mixes a section that improved with one that slipped.
The guessing rule
With five options, +1.5 for a right answer and −0.4 for a wrong one, a completely blind guess is worth slightly less than nothing on average. Rule out one option and a guess is worth making. The table is our arithmetic on the official marks.
| Options still open | Chance a guess is right | Average points per guess (ours) | Verdict |
|---|---|---|---|
| 5 | 20% | -0.020 | Leave blank |
| 4 | 25% | +0.075 | Answer |
| 3 | 33% | +0.233 | Answer |
| 2 | 50% | +0.550 | Answer |
The break-even is a 21.05% chance of being right (0.4 divided by 1.9): above it, answering is worth more than a blank on average. Your 2026 hit rate in each section, right answers divided by questions answered, shows how reliable your answers were there; where it was low, review why the wrong ones went wrong before you change how often you answer. Try your own numbers in the IMAT score calculator.
Past papers
Official IMAT papers from past years are worth sitting under timed conditions, and we do not sell them. Use only a copy published by the ministry or a university, and before you compare a score with a mock, check its number of questions per section against 4, 5, 23, 15 and 13. Our mocks are our own questions in the 2026 format, not CINECA questions, so they add to past papers rather than replace them.

Frequently asked questions
Is the IMAT 2027 format the same as 2026?
This guide uses the 2026 decree. In 2026 the test was 60 five-option questions in 100 minutes, marked +1.5, −0.4 and 0. Plan on that and check it against the 2027 decree when it is available.
How many more right answers do I need?
For the fewest, turn wrong answers right first at 1.9 each, and blanks at 1.5 each only if your wrong answers run out. For the most, fill your blanks first at 1.5 each, then wrong answers at 1.9 each. The free gap report on this page does it from your section numbers.
What was the cut-off at my university?
There is no single published cut-off. For non-EU candidates residing abroad, each university publishes its own ranking. The examples here come from the universities’ own 2025/26 lists; they are not this year’s cut-offs.
Should I guess on the IMAT?
Not blind: with all five options open a guess is worth −0.02 points on average. Rule out one option and it is worth +0.075; rule out two and it is worth +0.233.
Are the mini-mock questions real IMAT questions?
No. They are our own questions in the 2026 format, written to Allegato A, each checked independently twice before release.
